Harvest Craft Sale – Table Application

Cost per 6 ft. table is $15.00 (non-refundable). All tables will be filled and assigned on a first come basis. Tables may be shared.

This is 2-step process.

Step 1:

Complete the online application form below OR – download, print, and complete the following PDF.  Paper forms can be dropped off at Guild and Executive Board meetings.
All applications must be received by Elsa Shea by September 8.

Step 2:

Purchase the $15.00 table fee here OR – send a $15.00 check, payable to QGOTV, via your program rep or at a Guild and Executive Board meeting.
