Simply add the product (one or two times) to your cart but do not check out yet. Then click the back button (or back arrow) at the top LEFT of the computer screen (it may take two separate clicks, so be patient) to return to the application for review, signature and date... then submit it.

After application is submitted, then go back to your cart and complete payment.
